When using Qualtrics, there are a few things that we often see as challenges when setting them up. Use this template to start your project in Qualtrics and it will have some useful things already built in!

IRB Guidelines for Department
The department chair has to review each IRB submission and accompanying Qualtrics project prior to being submitted to the Human Subjects Research Office. The document below helps to provide additional information on what they are typically looking for when examining the proposals
Submission to Chair for review
When ready, the faculty member must submit their IRB materials to the chair. Please follow the link below to complete the submission process.
You will be asked to indicate that all steps have been completed and that you have shared the Qualtrics project with them. Your submission materials will be uploaded here as well.
